I'd suggest checking the wiki for thrall locations and going to these spots regularly. Personally, I found a T4 Blacksmith in two locations (multiple times). The first location is the Pagoda of Boundless Lusts. I also found one at Freya's Hovel (although that one seems very rare to spawn). Aside from that, some camps up North can spawn them.
